Consent for personal data processing (“Form “Leave your contact information”)

I, being a natural person and leaving a message on the website https://www.pureat.ru/ by means of the form “Leave your contact information”, acting voluntarily, according to my expressed will and in my own interests, and also acknowledging my full legal capacity, hereby give my consent for personal data processing (hereinafter – Consent) to the Operator: Pure Air Technologies Limited Liability Company (PSRN   1235000069820, TIN/TRRC (Tax Registration Reason Code) 5042162330/504201001, email address: info@pureat.ru, phone: 8-800-600-69-97 (hereinafter – Operator, NTT LLC), which is the owner of the website https://www.pureat.ru/ and is registered at the following address: 141320, Russia, Moscow Region, Sergievo-Posadsky Urban District, Kovrovo village 50, room 2.

Before giving this Consent to the Operator, I have read the Personal Data Processing Policy of the website https://www.pureat.ru/, located at the address: https://www.pureat.ru/, in compliance with which the Operator processes personal data.

The Consent is given on the following terms:

1. This Consent is given for personal data processing for the purpose of processing of my Application for receipt of a Quotation from Pure Air Technologies LLC, containing full information concerning the products and/or services which interest me.
2. The Consent is given for processing of my personal data as follows:

• Full name;
• mobile phone number;
• e-mail address;

3. Abovesaid personal data does not fall into any specialized categories of personal data and is not considered biometric personal data.
4. This Consent is given for personal data processing both with and without the use of automation means.
5. In the course of processing, the following actions will be performed concerning personal data: collection; recording; systematization; accumulation; storage; verification (update, alteration); extraction; use; transfer (provision, access); blocking; deletion; destruction.
6. Third parties processing personal data under order of Pure Air Technologies LLC for the said aim:

RSIC JSC, 125315, Moscow, Leningradsky Prospekt 72, building 3.
T2 Mobile Limited Liability Company, 108811, Moscow, Moskovsky settlement, Kievskoe Highway, 22^nd kilometer, estate 6, building 1.

7. Personal data is processed throughout the whole period of discussion of the project under my Application, and also within 30 days from the moment of refusal from the project or from the moment of making a decision to execute an agreement for the project, in accordance with pt. 4 of art. 21 of the Federal Law 152-FZ, whichever occurs first.
8. I understand that this Consent can be revoked by me or my representative by means of sending a request to Pure Air Technologies LLC in electronic form, signed in accordance with the legislation in the field of electronic signatures, to the address: info@pureat.ru marked as “Revocation of consent for personal data processing”.
9. This Consent shall come into effect from the moment it is given and until the moment of termination of personal data processing in accordance with clause 7 hereof.
10. I understand that in case of revocation of the Consent by me or my representative,
    Pure Air Technologies LLC shall have the right to continue processing of my personal data without my Consent in case there are grounds as specified in paragraphs 2-11 of part 1 of article 6 of the Federal Law No. 152-FZ “Concerning personal data” of 27.07.2006.
